Required clinicals: 

• Clinical Assessment Score (CAS)

• Labs that prove euthyroid or mild hypo/hyperthyroidism undergoing treatment to correct and/or maintain euthyroid

• Thyroid panel within 6 months of referral prior to initiation of therapy

• Hearing tests completed prior to treatment
