Required clinicals: 

• Negative TB screening

• Failure of at least one conventional therapy OR

• Documentation of contraindication to conventional therapy

• Normal liver enzymes (ALT/AST) and bilirubin (For Crohn’s and Ulcerative Colitis patients) prior to initiation of therapy
