Required clinicals: 

• Negative TB 

• Negative Hep B

• Positive for RF or anti-CCP OR Tested for all biomarkers but results negative (RF, anti-CCP, CRP, ESR)

• Documentation that patient currently taking methotrexate OR

• Documentation that patient tried/failed or has contraindication to methotrexate
