Required clinicals: 

• Clinical documentation supporting diagnosis of one of the following:

  • Severe asthma

  • Chronic rhinosinusitis w/ nasal polyps

  • Eosinophilic granulomatosis w/ polyangiitis

  • Hypereosinophilic syndrome

  • Tried and failed previous therapies

  • For Asthma indications only: Documented eosinophil count, pre-treatment ≥150 cells/mcL
