Required clinicals: 

• Elevated LDL-C (at least 190 mg/dL)

• Documentation that patient has tried/failed or has contraindication to high-dose statin therapy

• Documentation that patient has tried/failed or has contraindication to other conventional therapies

• ASCVD score
