Required clinicals: 

• Results of genetic testing to confirm dx

• Fibroblast growth factor 23 above upper limit of normal

• Radiographic evidence of rickets or other bone disorder consistent with XLH

• Documentation of current oral phosphate or active vitamin D analogs

• Serum phosphorus within 6 months of referral

• Serum creatinine within 6 months of referral
